2、手架排查情况： 本工程7#、8#、10#外墙装修正在施工中，外脚手架正在使用，存在错层施工情况，检查中发现存在错层施工高处落物隐患，现场记录，项目部立即采取模板在悬挑钢梁层进行全隔断封闭，限1周内完成，检查中发现8#楼19层个别悬挑钢梁木楔松动，有连墙杆被拆现象，现场责令架子工班组立即派人全面检查，并由安全员复查。 3、检查各分项、专项施工方案 经技术负责人全面清查资料，各专项工程施工方案齐全，已安装的施工机械备案手续齐全，维保记录齐全，维修保养情况属实，特种操作工人证相符。4、施工用电检查 检查中发现现场各楼层施工照明用电搭接混乱，有乱搭现象，现场责令电工立即拆除，并配备各楼层专用电箱，箱内
3、配专用插孔（多组）备用。 5、电梯作业部门检查 检查中发现6#楼、8#楼施工时有电梯离开后作业层门不关现象，当天开会，明确规定由电梯司机负责关门，并建立罚款制度，立即执行。 2013 福建九建建筑工程有限公司 阳光假日城二期项目部 年4月24日篇二：安全隐患排查及整改情况汇报 关于安全隐患排查及整改 情况汇报 医疗机构：景县安康精神病医院 院 长：徐汉臻 日 期：2014年1月10日 为了强化医疗安全，有效遏制安全事故的发生，深入贯彻落实李谦书记在中共衡水市委第三届委员会第五次全体（扩大）会议上对安全生产工作作出的重要指示精神，切实抓好“两会”及春节期间安全生产工作，我院高度重视，成立了医疗安
4、全隐患排查整治的工作专班，形成了职责明确、协调配合的组织领导机构，为医疗安全(转 载 于:wWw.zAIdian.cOM 在 点 网)隐患排查整治工作提供了组织保障，确保了长效工作机制的良好运行。工作专班及时召开会议，精心组织，认真贯彻落实，对医疗安全隐患各方面进行了一次彻底排查和全面梳理，并落实了相关整改措施，切实消除安全隐患，堵塞漏洞，减少和避免各种安全事故的发生。现将安全隐患排查及整改情况汇报如下： 一、安全隐患排查情况 （一）医疗技术方面 医院高度重视医疗技术临床应用，根据卫生部医疗技术临床应用管理办法，制定景县安康医院医疗技术临床应用管理办法实施细则（试行），认真落实医疗技术分类分级
5、管理，严格执行第二类、第三类医疗技术临床应用的管理，新技术的开展符合上级卫生行政部门和医院关于相关技术的审批、准入、应用、评价、医学伦理等相关规定；医院依法准予医务人员实施与其专业能力相适应的医疗技术；建立相应的设备、设施和质量控制体系，医务人员使用各类设备、设施严格执行操作技术规范，认真遵守技术管理规定。 （二）合理用药方面 我院严格遵守处方管理办法、麻醉药品和精神药品管理条例、抗菌药物临床应用指导原则、关于加强合理用药监测工作的通知和卫生部办公厅关于抗菌药物临床应用管理有关问题的通知，扎实开展抗菌药物临床应用专项整治活动，切实做到临床用药合理、安全、有效。制订医院抗菌药物供应目录，不定期对
6、抗菌药物目录进行全面梳理，清退存在安全隐患、疗效不确定、耐药严重、性价比差和违规促销的抗菌药物品种；严格控制抗菌药物购用数量不超过50种；加强抗菌药物购用管理，医院所有药物统一由药剂科从网上采购中标品种；制定抗菌药物分级管理目录和分级管理原则；根据致病菌种类及耐药情况，有计划地对同类或同代药物轮换使用。 （三）医院感染方面 各项管理及监测制度健全，并能根据新标准、新规范及时更新。各部门区域设置符合规范，标识清楚。手卫生设施齐全、运行正常，人员手卫生制度执行情况良好。血液分析室、消毒供应中心工作人员每年保存有体检记录，有传染病者不得从事以上两项工作。血液分析室的各项检测定时完成；传染病患者的管理
7、一律严格按照医院感染质量控制评价标准执行，每项内容有记录保存；消毒供应中心的灭菌效果的物理监测、生物监测、化学检测均符合要求，各项监测记录保存完整。医疗废物按照医疗废物管理条例处置，医疗废物交接、储存、转运管理严格，记录完整。此次排查过程中发现的安全隐患：少数医务人员手卫生依从性稍有欠缺。 （四）病原微生物实验室生物安全管理方面 检验科严格按照病原微生物实验室生物安全管理条例、人间传染的高致病性病原微生物实验室和实验室活动的生物安全审批管理办法及医疗机构临床实验室管理办法的有关规定，加强实验室生物安全、质量控制和全面管理。实验室严格执行全面质量控制，降低结果的误差，并实行危急值的报告制度，对特
8、殊异常标本进行全程控制，确保质量、安全。建立了生物安全管理责任制，成立了生物安全委员会，职责明确，并有明确的安全保卫措施。实验室实行准入制度，并进行定期的审查和修改，建立了实验室生物安全评估办法。实验室的人员需经考核合格才能持证上岗，建立了实验室技术人员技术档案和实验室人员健康资料。实验室标本的运送有严格的交接登记，有菌种及样本使用、销毁记录，菌种及样本的保存实行双人双锁。实验室配有生物安全柜、高压灭菌登记、感染性材料及废弃物运送容器等安全设备。实验室门上有生物安全警告标志，实验室分为清洗区、半污染区和污染区，但由于房屋面积限制分区不够规范。 （五）医院后勤治安保卫和消防安全方面 按规定配齐消
9、防栓、灭火器，消防重点部位，做到专人管理，专人负责。每周定期和不定期地全面检查消防安全，对容易引发火灾、存放危险品及人员集中的场所重点检查（如药库、病房、厨房、供暖室等），消防设施定期维护更换，确保消防器材的完好。对医院工作人员进行消防安全教育，普及用电、用气以及防火、灭火、火场逃生自救常识，努力提高自防自救能力。不定期抽查教职工，特别是氧气房和供暖室、厨房工作人员灭火知识，发现问题及时纠正。严格填写值班记录及交接班记录，杜绝重大可防性案件的发生。加强与公安部门的协调，制定人防、物防、技防应急措施，确保全院无重大责任事故。此次排查发现有些科室堆放杂物堵塞消防通道，责令立即整改。 库管根据各材料
10、的用途属性，按库房分类管理，及时清点、检查，做到日清月结。电工维修工作人员在带电作业或登高维修时必须有安全监护人，如需断电，电闸开关处必须留人看守，以防有人误送电危及人身安全。防止电路发生安全事故，定期对电路进行维修和保养。加强采暖炉安全管理，严格执行安全生产和操作规程，保证医疗和生活上的需要，准时生火，封火，做好节能工作。此次自查中发现：供暖室消防灭火器有轻微老化问题，责令马上更换。 二、整改措施 （一）提高认识，加强安全教育领导和管理工作。为进一步加强医院日常安全管理工作，牢固树立“安全第一”的思想，切实落实安全工作责任制，我院认真组织全院职工学习上级相关安全教育文件，并吸取近期其他地区发
11、生安全事故的教训，制定了相关安全工作措施以及相应的应急预案。 （二）加强对相关科室和人员进行安全教育和管理工作。我院按照文件精神进一步加强对职工的安全教育。通过板报、晨会等全方位进行安全教育宣传，全员树立安全意识，增强了各科室及人员的安全意识和自救自护能力。 （三）严格实行安全责任首问制。建立领导安全巡视制度和节假篇三：安全隐患排查治理情况汇报 橙子沟项目部安全隐患排查治理情况 根据分局安发201025号，关于进一步开展安全隐患排查地紧急通知文件精神，项目部认真开展了隐患排查治理专项行动，强化安全防范措施，坚决遏制重特大事故的发生，减少和排除一般事故的发生，实现分局下达的安全目标，项目部认真开
12、展了安全隐患排查治理工作。 一、指导思想 贯彻落实“安全第一、预防为主、综合治理”的方针，坚持以人为本的安全理念，落实项目部及所属各部门、各工区安全第一责任人的隐患排查治理责任，切实把隐患排查治理工作落到实处。 二、组织领导 项目部成立安全隐患检查领导小组： 组 长：乜海峰 副组长： 王江波、祁永贤、哈伟德 组员：樊光虎、冯文英、隆国苍、张庆红、董学臣 、赵亮、刘忠、林孝德、林立、林君民 三、安全隐患排查工作安排 1、进一步完善项目部及所属各部门及工区安全生产规章制度，根据施工现场的实际情况，组织制订安全操作规程并监督落实。此项工作由生产安全办进行负责落实。 2、明确项目部所属各工区的职业健康
13、安全目标、指标，签订职业健康安全责任书，保证安全工作落到实处。此项工作由生产安全办负责实施。3、项目部组织对机械设备尤其是特种设备进行检查，确保今年引水隧洞及崔家沟右半幅明挖段，开挖及浇筑的各类设备的安全运行，确保机械设备不“带病”运行。组织对现场施工用电进行专项安全检查，督促安排专人对台车作业人员及崔家沟眀挖段边坡底作业进行现场监督。此项工作由设备物资、安全保卫部门及各工区电工负责落实。 4、检查危险爆破物品的运输、储存、保管、使用等情况，严格执行国家和上级有关部门制定的相关安全操作规程。此项工作由生产安全办负责落实。 5、继续组织开展三级安全教育、外协队伍安全教育、特种作业培训工作，确保特
14、种作业人员全部持证上岗，确保各项目部及外协队全体员工都能参加安全培训。此项工作由生产安全办、综合办负责实施。 6、每日对引水隧洞前期塌方处理部位、重大危险源进行监控，及时要求施工队伍支护跟进掌子面，对锚杆、钢支撑进行质量检测，锚杆长度按照设计要求进行落实。此项工作由生产安全办及技术质量办督促落实。 7、根据施工进度，进行专项、专业检查，及时排查出施工现场存在的危险点（源）并督促落实整改。此项工作由生产安全办及工区负责人监督落实。 8、重点抓好施工现场安全帽、安全带及安全绳的正确佩带使用工作。此项工作由各工区、各部门及生产安全办负责实施。 四、隐患排查情况1、引水隧洞1#支洞段上、下游及2#支洞
15、段上游岩石为石英云母片岩，且岩体破碎，属类围岩，开挖爆破后清面出现塌方及掉块现象，存在安全隐患。 2、引水隧洞1#洞渣场弃在1#砂石骨料系统渣部位途径212国道、外纳中学，车辆较多，遇学生放学及外纳赶场时，人员较多，易出现较大交通事故； 3、项目部生活营地邻白龙江边板房后未加设防护围栏，且板房后窗极易开启，当地学生经常在此部位出现，易发生盗窃现象； 4、引水隧洞（2#支洞段）上游引4+1104+130段因岩体应力释放，钢支撑受压出现扭曲变形现象，易出现塌方、掉快现象，存在较大安全隐患； 5、崔家沟眀挖段基坑开挖边坡部位，存在两块较大危石，桩基施工时震动较大，易出现滑落现象，容易对基坑施工人员造
16、成伤害。存在较大安全隐患； 五、隐患治理情况 1、引水隧洞1#支洞段上下游及2#支洞段上游岩石为石英云母片岩，且岩体破碎，属类围岩，开挖时采用弱爆破、短进尺、强支护的开挖方式，要求工区施工人员爆破清理浮石及危石后进行支护，支护及时跟近掌子面，防止坍塌、掉块的现象发生，确保了人员、设备安全； 2、引水隧洞1#支洞弃渣场在1#砂石骨料系统渣部位途径212国道、外纳中学，项目部在每次周例会要求工区错开学生放学时间及赶集时间，要求出渣车辆司机限速行驶，出渣时间尽量调整至晚上，车辆和人少时进行出渣，防止了交通事故的发生； 3、项目部对生活营地邻白龙江挡墙部位安排人员加设防护围栏，要求宿舍人员人不在时，锁
17、好门窗，并留守值班员进行值班，防止盗窃事件的发生； 4、引水隧洞（2#支洞段）上游引4+1104+130段因岩体应力释放，钢支撑受压出现扭曲变形现象，项目部安全员加强对此部位安全监控，待定出处理方案后进行处理，确保工程、人员安全； 5、要求现场安全员加强对崔家沟眀挖段边坡危石的监控，定期联系测量队进行安全监测，确保施工人员安全； 项目部高度重视本次安全生产隐患排查治理工作，把安全生产隐患排查治理工作做实做好，安全管理部门做到检查不走过场，不流于形式，严细认真，全面到位，突出重点，实现了安全检查“无盲区、无死角、无漏点”的三无管理目标，及时的将事故隐患消除在萌芽状态，有效的保证了全项目部良好的安
